Africa's AI Health Tools Thrive on Donor Funds, Struggle for Self-Sufficiency

Artificial intelligence tools are increasingly being deployed across African health supply chains, bringing automation to critical areas like procurement planning and fraud detection. However, a new study by Salient Advisory, funded by the Bill & Melinda Gates Foundation, reveals a significant challenge: nearly all of these 20+ identified AI-enabled solutions remain heavily dependent on donor funding, lacking clear pathways to financial sustainability.
The research highlights that of five in-depth case studies, only one directly generates revenue from its users. The vast majority are propped up by grants from foundations and development financiers, raising concerns about their long-term viability once external support diminishes. Furthermore, the report notes a critical gap in understanding cost savings, with most impact figures being self-reported by providers and not independently verified.
Examples of these tools span the continent, from an Ethiopian platform claiming $38 million in procurement savings to a Kenyan system that drastically cut planning time, and a Nigerian agent that reduced invoice-to-pay cycles. In Morocco, a private-sector inventory management service reported a 20% reduction in pharmacy stock levels. Yet, even successful deployments like Ethiopia's ForLab Plus and Kenya's SMArT tool are financed by grants, with the hope of future government payments that remain uncertain. A Ghana proof-of-concept, designed to improve supply chain visibility, has already paused due to lack of funds.
The sustainability issue is compounded by a general absence of hard data on cost savings, with many solutions emphasizing operational gains over verifiable financial impact. This situation is particularly precarious as official development assistance for health is declining globally. The report urges solution providers to develop data-backed business cases, global health organizations to negotiate equitable access to AI infrastructure for African innovators, and governments to conduct sober assessments of procurement options, establish cost baselines, and ensure AI insights genuinely inform planning.
Despite these cautionary findings, the research indicates that AI is maturing beyond pilot phases into operational use in Africa, with 60% of identified solutions exclusively in health supply chains and a third of providers headquartered in emerging markets. The report concludes that coordinated action is essential to build on these early gains and prevent promising AI tools from becoming another statistic of short-lived, donor-funded digital health initiatives.
